WHOSE HISTORY
NATIVE FAMILIES' PERSPECTIVES OF HISTORY LESSONS
Whose History is a project working, in part, to better understand how Indigenous perspectives are represented in K-12 classrooms. We would like to include perspectives of students and families in the Title VI Program. We’d like to invite your family to share your perspectives on what students are learning in classrooms, how this affects them, and things they may want to learn.

We need one Adult Form for each adult to participate.
Adult Form
This form is for people 18+ who want to participate in the research. Each individual person who wants to join needs to complete your own form.
We need one Guardian Form and one Student Form for each student to participate.
Parent/Guardian Form
This form is for parents/guardians to complete if you want your student(s) to participate in research. You can fill out one form for multiple students.
Student Form
This form is for students to complete if they want to participate in research after their parents say it is okay. Each student should complete their own form.
